The challenge

Achieving better health outcomes and lowering costs require addressing related social barriers. To tackle this, New York State established Social Care Networks (SCN) to provide Medicaid members with food, transportation, and housing solutions.
 
When selected as the SCN lead entity in New York City, a leading healthcare organization called upon Eve & Co to build a network of community health providers to which it could allocate funds for greatest impact.

The solution

To structure the network efficiently, the client needed a nuanced understanding of the two main challenges: service delivery and access to care.
 
We began with service delivery, hosting small group listening sessions to hear firsthand from 96 community health providers about the challenges they face. We then used those insights to build a capabilities survey of service providers, which was distributed to over 100 organizations to assess which were the best fit.
 
To address access to care, we hosted multilingual discussions for Medicaid members and invited their feedback on solutions.
 
We combined the service delivery and access findings into a recommendation for the network structure and makeup, prioritizing expertise, implementation speed and efficiency, and geographic coverage.

The impact

Eve & Co’s recommendation formed the basis of the client’s governing board report, which laid out the deployment of $152M over three years. It used survey results to detail specifics for capacity building, training, technical assistance, and delivery improvements.
